How does your locality memorialize conversations from closed session evaluations by the board or council (clerk, attorney, administrator/manager – anyone appointed/reviewed by the board or council) if minutes aren’t kept of the closed session? Does your board/council submit written comments/ratings? If so, could you please give an example of a form?

Comments

Charter employees are provided the same employee evaluation form as city employees, and that is what is used in the closed meeting and also put in their file. No other records of the actual meeting are kept.

An evaluation form is prepared regarding the employee's performance for the year, discussed in closed session but not put in the minutes. A personnel resolution is prepared after the meeting noting items voted on in closed meeting

I like the cut of your job :) or at least your thought process but sorry to say, I honestly think you would have fully sold me on the idea had you been able to back up your premis with a substantial bit more solid facts.
